The form must be completed and postmarked by March 28, 2025.  

Refunds are issued as below excluding $25 processing fee.

Cancellation refunds are issued as follows:

 

• Prior to January 1, 2025, receive 75% refund.

• From January 1 to February 15, 2025, receive 50% refund.

• From February 16 to March 28, 2025, receive 25% refund.

• No refunds will be issued after March 28, 2025.  Please be sure to carefully check your schedule.
